Ballarat Health Services is seeking a dynamic, energetic Registered Nurse to join the Transition and Restorative Care Team as a Care Coordinator.
If you have exceptional clinical skills, a commitment to quality and are a driven leader this is the position for you.
Working within a multidisciplinary team you will be expected to facilitate and foster best practise and service delivery to our clients and their families within the Transition and Restorative Care Programs.

The successful candidate will have knowledge and experience in the areas of assessment, chronic disease management, My Aged Care processes, psychosocial, cognitive and mental health conditions.
